Dear Parents/Guardians,

It is that time of the year again to begin football physicals.  Prior to participating in any athletic try-outs, practices, voluntary workouts or games students must have:

1.      A certificate of an annual physical examination on file at the school. Physical examinations will be good for twelve (12) months from the date of the exam.

2.      A completed FCBOE Parental Consent Form

3.      A signed GHSA Concussion Awareness Form

4.      A signed FCBOE Code of Conduct Contract
